Shoulder Pain Is Quietly Ending Climbing Careers — Here's What You Need to Know

Ask any experienced climber about finger injuries and you'll get a twenty-minute lecture. Ask about shoulders and you'll usually get a shrug — which is ironic, because that shrug might actually hurt them a little bit.

Shoulder problems in climbing are everywhere. They're just not talked about. Pulley strains get the Instagram posts, the commiseration threads on Reddit, the carefully documented recovery timelines. Meanwhile, rotator cuff issues and shoulder impingement quietly accumulate in the background, dismissed as "tightness" or "just how it feels after a hard session" — until they aren't so easy to dismiss anymore.

"I kept telling myself it was normal soreness," says Marcus, a 34-year-old sport climber based in Chattanooga who eventually needed six months off after ignoring escalating shoulder pain for nearly a year. "By the time I actually saw someone, I had a partial rotator cuff tear and impingement in both shoulders. I'd been climbing through it the whole time."

He's not alone. Sports physical therapist Dr. Renee Harlow, who works with climbers at a practice in Asheville, North Carolina, says shoulder complaints are among the most common issues she sees — and the most preventable. "Climbers spend a lot of time pulling," she explains. "And the shoulder is not naturally designed for the volume and angles that climbing demands. Without deliberate attention to balance and stability, you're setting yourself up for trouble."

Why Climbing Is Uniquely Hard on Your Shoulders

To understand the problem, it helps to understand what the rotator cuff actually does. It's a group of four muscles — supraspinatus, infraspinatus, teres minor, and subscapularis — that work together to stabilize the head of your upper arm bone inside the shoulder socket. They're not the big movers. That's the deltoid and the lats. The rotator cuff's job is to keep everything centered and controlled while those larger muscles do their thing.

Climbing puts this system under constant, asymmetrical stress. Overhanging routes load the shoulder in positions it wasn't built to sustain repeatedly. Lock-offs — holding a bent-arm position under load — compress the joint. Dynamic moves create sudden force spikes. And because most climbers pull far more than they push, the muscles responsible for external rotation and scapular stabilization get progressively weaker relative to everything else.

Impingement happens when the tendons of the rotator cuff get pinched between the bones of the shoulder, particularly during overhead movements. It starts as a dull ache, often on the front or side of the shoulder. It gets worse with activity and better with rest — which is exactly why it's so easy to rationalize away.

"The insidious thing about impingement is that it gives you just enough relief between sessions to convince yourself it's not a real problem," says Dr. Harlow. "And then one day it doesn't go away anymore."

Red Flags You Shouldn't Ignore

Not every shoulder ache is a crisis. But some signals are worth taking seriously:

Any combination of these warrants a conversation with a sports medicine professional or physical therapist — ideally one who has experience with climbing or overhead athletes. Don't wait until you can't raise your arm.

The Prehab Routine That Actually Fits Your Life

Here's the good news: most shoulder issues in climbers are preventable with consistent, targeted work. The even better news is that you don't need to add an hour to your gym sessions. This routine takes about 12–15 minutes and works best done before climbing as part of your warm-up, or on rest days.

1. Band Pull-Aparts — 3 sets of 15 Hold a light resistance band at shoulder height with arms extended. Pull the band apart until your arms are fully spread, squeezing your shoulder blades together. Slow and controlled beats fast every time here. This directly targets the posterior shoulder and scapular retractors — the muscles most neglected by climbing.

2. Side-Lying External Rotation — 3 sets of 12 per side Lie on your side with a light dumbbell (2–5 lbs is plenty). Elbow bent at 90 degrees, rotate your forearm upward against gravity. This isolates the infraspinatus and teres minor, which are critical for shoulder centering. Go light. This is not a strength exercise — it's a stability exercise.

3. Wall Slides — 2 sets of 10 Stand with your back against a wall, arms in a goalpost position, forearms and elbows touching the surface. Slowly slide your arms overhead while keeping contact with the wall. If your lower back arches or your elbows lift off, you've found your mobility limit. Work within it.

4. Prone Y-T-W — 2 sets of 8 each position Lie face down on a bench or the floor. Move your arms into Y, T, and W shapes, lifting them off the ground with control. No weight necessary — or a very light plate if you're well-conditioned. This hits the lower trapezius and serratus anterior, both essential for healthy scapular movement.

5. Sleeper Stretch — 2 minutes per side Lie on your side with the target shoulder down, arm extended at shoulder height, elbow bent 90 degrees. Use your opposite hand to gently press your forearm toward the floor. This stretches the posterior capsule, which gets chronically tight in climbers and contributes directly to impingement.

Making It Stick

The hardest part of prehab isn't the exercises — it's the consistency. Marcus, who now does a version of this routine three times a week without fail, says the shift was treating it like training rather than recovery. "I used to think of it as something I did when I was hurt," he says. "Now it's just part of climbing. It's what lets me keep climbing."

Dr. Harlow recommends attaching the routine to something you already do. "Do it while you're watching video of a project, or right after you change into your climbing shoes. Make it automatic."

Your fingers get all the attention. Your shoulders do just as much work. It's time to treat them accordingly.
